The finding of hyperplastic lymphoid tissue in patients with Graves' disease is not uncommon. However, massive thymic enlargement is quite rare. In this report we present evidence that a massively enlarged thymus in a patient with Graves' disease returned to normal upon institution of propylthiouracil therapy. We suggest that a potential response to antithyroid therapy must be considered before recommending invasive procedures.
